Appointments
To schedule an appointment, Call the office at 302-858-4381 and our friendly staff will be happy to schedule your appointment.
Insurance
Bring your insurance card to every visit if you are covered by insurance. If your Insurance has changed, please notify us before your next appointment so we can verify with the insurance company any changes in your copay or deductible.
Prescriptions
Call your pharmacy first to see if they have refills available for your medication so that the pharmacy can directly get the medication ready for you. If you need a new prescription to refill your medication, please call the office and leave a message on our prescription line. Please leave the date you called, your full name, your date of birth, the name and dosage of the medication and the name of your pharmacy. Please allow 72 hours for us to send it to the pharmacy.
Things to Bring to Your Appointment
Please bring with you your mask, a valid ID (for example a driver's license), most recent insurance card (if applicable), and bring your medications. We collect copays and the cost of your visit at the time of your visit.
Forms You Need Filled
If you have a form that you want filled by the office, please carefully review it and complete the parts you are responsible for prior to your visit. Please let staff know you have a form that needs to be completed when you are signing in for your visit and before the visit starts. Some forms may take up to 5 days to complete.
Tardiness
Arrive 10 minutes before your scheduled appointment. Please let office know if you are going to arrive late and past your scheduled time. If you arrive more than 15 minutes after your scheduled time, your appointment may be rescheduled and considered a no show.
Missed Appointments
We require 24 hour notice if you cannot keep your appointment. If you miss your appointment without notice, there may be a $25.00 charge (which is not covered by insurance companies). This is also considered a NO SHOW! It is your responsibility if there are special circumstances to notify our office.
Missed Appointments Without Notice
If you miss 3 or more appointments (three no shows) in one year without proper notification, you will be discharged from the practice and your chart will be closed. We have a waiting list of people who would like to be seen in our office.
